• Awardees will be notified by 5:00 pm on Monday, September 13, 2021.

Instructions for Scholarship Application Forms

• The Scholarship Application Form must be submitted to Matthew Gower at Division of Behavioral Health and Recovery, no later than 5:00 pm on August 20, 2021 at 5:00 pm.

  You will be notified via email no later than Monday, September 13, 2021 regarding your scholarship status.
• Scholarships are first-come, first-served basis to those who meet the criteria.

Due to the large number of applications submitted, late applications will not be considered.
