About this Post-Conference Institute:  The state’s new Foundational Community Supports Medicaid Transformation (also known as the 1115 Waiver) provides opportunity to further advance recovery among individuals with behavioral health conditions through the delivery of evidence-based supportive housing and supported employment services. This full-day post-conference institute will highlight the state’s vision for how these Foundational Community Supports will strengthen the system and increase opportunities for integrated community living and employment. Participants will learn about models for implementing supportive housing and supported employment services, and the impact on health and recovery outcomes, from both the participant and provider perspectives. Attendees will have the opportunity to partake in interactive breakout sessions for new and more experienced providers on topics such as core competencies for delivering and /or integrating supportive housing and supported employment services, and partnership opportunities between behavioral health providers and housing, homelessness, and employment systems.
